This form must be signed by all members of your group age 18 and older.
We require each group to submit an updated medical form annually.
(REQUIRED for unaccompanied minors)
This form must be completed by a parent or legal guardian in order for your minor child to attend Cazadero Family Camp. Guardians must be at least 21 years old. All camp guardianships are subject to approval by the Cazadero Performing Arts Family Camp management.
In 2017, the community agreement is NOT a required form. Instead, we are offering the community agreement as a tool to help your family (especially teenage campers) understand our community expectations.
